- Addition of 3 measures
- Discharge to Community Post-Acute Care (using 2 years of claims data)
- Looks at the amount of patients who do not go to the hospital or pass away within 31 days after leaving home health
- Potentially Preventable Hospitalization (PPH)
- Evaluate and lower potentially preventable hospitalizations while receiving home health care
- Discharge Function Score
- Relies on GG items and compares discharge OASIS with Start of Care or Resumption of Care OASIS
- Will account for 20% of your total performance score
- Discharge to Community Post-Acute Care (using 2 years of claims data)
- Changes
- Improvement in Dyspnea and Improvement in Management of Oral Medications will have a weight impact change on the overall total performance score.
