What Auditors Are Actually Finding: Therapy Documentation Failures in the Era of Targeted Review

Speaker: Natalie Venable, OTR/L, MOT with Thrive Healthcare Solutions

Three documentation patterns are appearing repeatedly in current home health therapy denials — and most agencies don’t know they’re exposed until the demand letter arrives.

Federal audit activity targeting home health therapy services has intensified, and the documentation gaps examiners are finding aren’t new. They’re the same vulnerabilities that have existed for years: poorly justified maintenance programs, missed or inadequate 30-day reassessments, and therapy goals that describe tasks instead of medical necessity. What has changed is the precision and frequency of targeted review — and the financial consequence of being caught unprepared.

This session translates the current targeted review landscape into concrete, clinician-level action. Drawing on real-time findings from active audits, attendees will leave knowing exactly where their documentation is exposed and what to do about it before a reviewer gets there first.

Upon completion of this session, participants will be able to:

  1. Identify the documentation elements required to establish and sustain medical necessity for maintenance therapy programs under current CMS guidance and targeted review criteria.
  2. Recognize the most common deficiencies in 30-day reassessment documentation and apply a compliant structure that satisfies both clinical and regulatory standards.
  3. Construct therapy goals that demonstrate skilled need, functional relevance, and measurable progress — the three components most frequently cited in claim denial rationale.
  4. Evaluate your agency’s current therapy documentation practices against active audit criteria to identify systemic risk before a targeted review request is received.

Speaker Spotlight:

Natalie Venable, OTR/L, MOT is the CEO and Founder of Thrive Healthcare Solutions, a home health and hospice consulting firm based in New Braunfels, Texas. With a clinical background in occupational therapy and executive experience directing clinical excellence across large multi-branch home health organizations, she brings a rare combination of frontline documentation expertise and systems-level risk analysis.

Natalie holds certifications in COS-C, CHS-C, AFC- CP. She is a CHAP Certified Consultant and Certified Lean Healthcare Provider. She has contributed courses to Medbridge, partnered with CHAP for workshops, and consults actively on OASIS accuracy, PDGM, HHVBP/Star Ratings, federal audit response, and QAPI program development. Her consulting work provides direct access to the real-time denial patterns that form the foundation of this session.

Speaker: Michael Tidd, COS-C, HCS-D with HealthCare Synergy, Inc.

As AI becomes more common in Home Health and Hospice, agencies must balance efficiency with compliance and documentation integrity. This webinar will explore practical uses of AI, regulatory considerations, and safeguards agencies should understand to help protect reimbursement and reduce risk.

Speaker Spotlight:

Michael Tidd has over 20 years of experience in the Home Care Industry and is a frequent speaker at HealthCare Synergy Workshops and Webinars. He is a Software Developer, LVN, and Certified in ICD-9/10 Coding and OASIS (COS-C, HCS-D). As the Clinical Manager for HealthCare Synergy, he directs the Outsourced Management Services, assists in regulatory requirement of Synergy EMR, oversees the Clinical Documentation Auditing provided to clients, and participates on the clinical board for Synergy EMR. Michael continues to educate agencies on the ongoing regulations that CMS produces, the proper application of the Medicare Conditions of Participation, and completing timely, accurate, and defendable clinical documentation to survive in the current Medicare Reimbursement Model.

Revenue Cycle Optimization

Speaker: Scottie Strong, Founder of Strong Consulting Group Services

This session helps home health and hospice agencies evaluate how revenue cycle performance begins before admission and continues through referral acceptance, NOA/NOE timeliness, clinical documentation, billing review, LCD awareness, and claim follow-up.

Upon completion of this session, participants will be able to:

  1. Assess referral acceptance decisions using staffing, clinical capacity, payer requirements, financial exposure, and hospice cap awareness.
  2. Explain the operational and payment impact of late NOA and NOE submissions, including hospice non-covered days and cap implications.
  3. Identify practical points of interaction between intake, clinical, authorization, QA, and billing teams before claim submission.
  4. Develop a revenue cycle monitoring process that reduces denials and prevents money from being left on the table.

Speaker Spotlight:

Scottie Strong is the founder of Strong Consulting Group Services and has served the home health and hospice industry since 1993. His work focuses on billing, administrative operations, Medicare reviews, compliance strategy, documentation, and agency workflow improvement.

Scottie has represented clients in well over 1,000 Administrative Law Judge hearings and has assisted with approximately 1,500 to 2,000 ADR reviews. He has also helped agencies respond to Focused Medical Review and develop strategies to return to a stronger compliance position.

Scottie also brings decades of experience working with Healthcare Synergy as an office automation consultant, sales consultant, and software design consultant, using the software as a tool to improve agency workflow, billing, documentation, and administrative efficiency. When OASIS was introduced in 2000, he worked with the State of California Department of Health Services to provide regional provider education on proper OASIS completion.

ADR Appeals & Response Readiness
