Part 2/3 – Documentation that Defends: Navigating Conditions of Payment & Medical Review with Confidence
This webinar dives into the critical connection between documentation, Medicare payment requirements, and the medical review process. Participants will gain clarity on what is required to establish eligibility and support payment for skilled services, particularly during audits and reviews. We’ll explore common documentation pitfalls, how to address them, and practical ways to ensure clinicians and QA teams are capturing the right information—accurately and defensibly.
Objectives:
• Define the Conditions of Payment and their impact on Medicare reimbursement.
• Understand the triggers and types of medical review (e.g., ADRs, SMRC, UPIC).
• Analyze common reasons for claim denials and how to proactively address them.
• Identify documentation elements needed to support skilled service eligibility.
• Strengthen clinician and QA staff practices to improve documentation defensibility.
